Anytime you get hurt at work it's a worker's comp injury.
Our union always told us if you get hurt at work, make them call an ambulance no matter how small the injury. The reason for that is that then there is a record that is out of control of the company that you were picked up at your place of employment so then the company cannot dispute that it happened on the job.

if the injury is your fault then i dont think they cover that.

The injury must be due to employment. Fainting and striking your head, tripping on your shoelaces, and getting a heart attack while sitting at your desk are probably not covered.
If you fainted and fell into some machinery or got a heart attack while lifting bags of cement probably would be covered. It is hard to tell where the exact line is.

Anything that requires the use of special protective gear that is provided to the employee by the employer. If the employee does not use such gear, by his own choice, then he/she will suffer a reduction in benefits and may lose all benefits. Also, in most states a positive drug test gets rid of all workers comp benefits, regardless of the situation in which they occurred.
