file with state comp board a report as well as your employer

Your state may have a workers' compensation commission - call them. Ordinarily a claim for compensation is filed with them first. You should be able to find an attorney that works on a contingency fee (no fee unless you win).

If you're an employer, contact your workers' comp. insurance carrier.
